REQUIRED FORMS
Please remember that a current physical and STU health form must be approved by and on file with the school nurse before beginning practice. If last year’s physical is less than a year old a student may be allowed to begin practice but a new physical will be required before it expires. Forms are available online under the admissions tab-enrolled student forms section of our website. You are encouraged to turn in forms in advance but may bring these to the meeting. Parent Athlete Contracts will be provided and must be signed by parents and athletes and returned by the second day of practice.

FIFTH GRADE ELIGIBILITY PILOT PROGRAM
Stuart Hall School will conduct a one year pilot program during the 2011-12 school year which will allow the option for fifth grade students to participate on middle school athletic teams. The process will include a three to five day trial period of practices followed by a meeting of the parents with the coach of the sport to discuss the student’s readiness for inclusion in the program. Parents will be responsible for transportation and expenses related to participation and compliance with policies and procedures as stated in the athletic handbook. The process does require the same medical forms used for the middle and upper school which may be accessed as described above.
